The reason fluoride is used is because fluoride is a negative ion that can bind to the apatite - also fluorapatite dissociates at a lower critical pH. Enamel remineralisation is the first step in reversing carious lesions. Once they have cavitated though, fluoride isn't going to work - you will need a filling
