If you have surgery and don't take fin your pre-transplant hair will continue to fall out and it will look patchy. If you're going bald then there's no scenario where you get to keep your hair and don't have to take fin. I started taking fin when I was 26, it slowed down the hair loss but didn't grow back anything I already lost so unless you start taking it when you basically still have your original hairline you'll just be holding it off.

Those same men would be visibly balding during early 20s anyway, and getting a gf when you're 18 then losing your hair is a good way to get brutally dumped. Hair transplants exist and finasteride / minoxidil / ketoconazole shampoo will let you keep your hair as long as you start early, but consider if it's worth it because men shouldn't really take fin. I started losing my hair when I was 20 and I've still fucked 30+ women, sure it's harder with less hair but not impossible.

It's caused by over sensitivity to dihydrotestosterone, best combo that will stop it completely and can reverse it if you catch it early enough is finasteride tablets 1mg a day, ketoconazole shampoo, and regaine / rogaine. Things like saw palmetto also help block conversion of test into DHT. Good intake of vitamin D, zinc, biotin and selenium will also help.
