Scores are the lowest form of rewarding the player for accomplishing goals, and it's done simply by keeping track of how many times the player hits the goals. His point was that this is an easy way to make a game, answering the OP question of what it takes to make one.

SMB has different ways to score, namely finishing the level. But then you're looking at triggers and level transitions, which is already more complex than a simple scoreboard.
